Moving from children's social care to adult's social care

Until the age of 18, services for children and young people with long-term health conditions are provided by child health and social care services. From 18, they’re usually provided by adult services.

Between the ages of 16 and 18, the child will start a “transition” to adult services.

This should involve all the services that support areas like:

  • health and social care
  • mental health
  • education
  • financial benefits for the young person and their family
  • work
  • housing
  • Planning for this transition should begin when a child is in Year 9 at school (13 or 14 years old) at the latest.
  • Transition should be an ongoing process rather than a single event and tailored to suit the child’s needs.

Annual Health Check

As you get older it is important to stay fit and look after yourself. If you have a learning disability, poor mental health or physical health your GP will ask to see you once a year. The Annual Health Check scheme is for adults and young people aged 14 or above with learning disabilities that need more health support and may otherwise have health conditions that go undetected.

Learning Disability Register

The learning disability register is a list of people with a learning disability that the doctor’s surgery looks after.

Every doctor’s surgery has their own learning disabilities register.

The learning disability register list will help the doctor, nurses and other healthcare staff know what support you need.

Adult Autism Service

The Adult Autism Spectrum Disorder (ASD) Service provides support to adults (18+) with suspected or diagnosed autism. The team consists of a multidisciplinary team of professionals, including Psychiatrist, Clinical Psychologists, Assistant Psychologists, Occupational Therapists, and Sensory Integration Specialists who will be involved as per the clients need.

Adult Speech and Language Therapy (SALT) Service

The adult speech and language therapy (SALT) service supports clients with acquired communication and swallowing difficulties. The team works closely with carers and other staff groups, such as doctors, occupational therapists, nurses and dieticians, to offer assessment and advice.

Barking & Dagenham Mental Health & Wellness Teams (MH&WTs)

The Mental Health & Wellness Teams continue to provide specialist mental health services for adults aged 18 and over with serious mental health problems. Each team is a multi-disciplinary service made up of community mental health nurses, occupational therapists, psychiatrists, psychologists, and support workers.

Community Learning Disability Team

The community learning disabilities team is an integrated health and social services team. We offer a service to adults aged 18 and over who may need care and support because of a learning disability. The team can help those who have a learning disability live in Barking and Dagenham aged between 18 and 65 and a young adult coming into adulthood. The CLDT team will carry out an assessment to see what kind of care and support may be needed.

Psychological Services

Barking and Dagenham psychological services provide psychological assessment and therapy for adults aged 18 and over.
People experiencing mild to moderate depression and anxiety are seen in our Improving Access to Psychological Therapies (IAPT) services.

For people requiring multi-disciplinary assessment and input, psychological therapies are accessed via secondary care mental health services. People with a first experience of psychosis under the age of 34 years can access psychological help through the Havering, Barking & Dagenham Early Intervention Service.

Talking Therapies

It can be difficult having to cope alone and it is often impossible to see a clear way through. Whether your issues are caused by problems you face every day, or by events that have happened in your past – talking therapies can help. They are a free and confidential NHS service that will work with you to help you feel better.
